The Pikes Peak Personal Networks for Transition Project

Please help PEAK Parent Center get the word out to transition-aged youth and young adults with disabilities and their families in Colorado Springs and the Pikes Peak region!  The Pikes Peak Personal Networks for Transition Project is an eight-month program that begins in April, 2021 and is designed to help participants plan for the future in a positive, strengths-based way.

Program components:

• Training on how to build and maintain supportive networks during transition.

• Individual assistance from trained personal network facilitators.  

• Development of individual asset-based plans for the future.
